Altera Evaluation license. Please, fill
in the blank in English.

When you need Max+Plus II, Quartus II, you should give us HDD Number(Hard Disk Serial Number).

When you need Modelsim,
you should give us NIC Number. |
| 3. |
You should choose only
one between VHDL and Verilog when you use
Modelsim. |
| 4. |
Workstation users should give us HOST ID and mention how many copies you need. |
| 5. |
You can use evaluation license for 3 months and extend only one more time. |
